OMB Accreditation Empty OMB Accreditation

Mga Requirements:

  1. Letter of intent address to the Chairman , klangan nakasaad ang pakay na you are applying for accreditation as an Internet access service provider
  2. Duly filled-up application form (3 copies) must be notarized
  3. Pagawa ka sa lawyer ng Affidavit of undertaking na kung saan nakasaad na wala kang CD burner sa shop at hindi ka engaged sa CD burning services at selling ng mga optical media products.- must be notarized
  4. Four (4) pcs. 2X2 photo, signed by the applicant (hindi po sila tumatanggap ng scanned pictures)
  5. Picture ng harap ng shop na may kitang signboard (kung wala kang signboard, pagawa ka ng tarp o kahit cartolina man lang na may pangalan ng shop at idikit mo sa pintuan bago mo picturan.
  6. Picture din ng loob ng shop
  7. DTI cert. of registration para sa single prop./SEC cert. naman sa partnership o corporation
  8. Location sketch (klangan may street signs at land marks) pede nman daw kahit drawing o gawang kamay lang
  9. Floor lay-out (klangan kita ung entrance, saan cashier, mga computer, any rooms at mga lagusan)pede kahit drawing o gawang kamay lang
  10. Isulat sa isang papel ang TIN ng negosyo o ng may ari ng negosyo
  11. Table of organization- sa single prop isulat lang ang pangalan ng owner at ng mga staffs with
  12. Software Inventory ( Microsoft Internet Café Rental Agreement) Xerox copy lng ok na kapag may number ka na at pirmado na ni MS
  13. List of Computers
Halimbawa:
Computer No. 1
AMD Athlon 64 X2 5000
Microsoft Windows XP Home edition
Microsoft office 2007 (Kung meron lng)

Kailangan supported ng Xerox o scanned ng MS sticker bawat isa
pede naman burahin ung CD Key

Mga iba pang bagay na pede kailanganin:
Special power of Attorney- Kung wala ung signatory
Authorization letter – kung wala o hindi available ung may ari sa pag received ng certificate

Mga Hakbang:


  1. Kapag kumpleto na ang mga kailangan, Tumawag sa OMB at alamin ang schedule ng orientation seminar. Required po kasi na umattend muna ng orientation seminar bago makapag apply ng accreditation. Bayad po dito sama na certificate of attendance = 50 pesos.
  2. Umattend ng Orientation seminar at dalhin na lahat ng mga requirements.
  3. Pagkatapos ng seminar, ipasa anag Letter of intent, klangan pa kasi ma-recieve at ma aprub ni chairman o authorized personel bago ma process application.
  4. Kapag aprub na punta ka sa Legal department (for Clearance) bayad 50 pesos
  5. Kapag cleared ka na saka pa lang nila tatanggapin ung mga requirements
  6. kapag kumpleto, bayad ka 500 pesos pa ra sa accreditation fee.
  7. After makuha muna ung resibo pede ka na bumalik hanggang 15 days daw para makuha mo na ung certificate mo.
  8. Better call muna bago punta, just to be sure.
Mga gastos at bayarin:

  • Orintation Seminar 50
  • Clearance 50
  • Accreditation fee 500
  • Notary: application form 200
  • Affidavit plus notary 300
  • 2x2 picture 60
  • Picture: façade at inside 60
  • Misc.: Xerox, bond paper, ball pen atbp 50
  • Pamasahe/pagkain/palamig (300x2) 600
